PROJECT/WORK DESCRIPTION: Furnish all labor, material and equipment required to accomplish the construction described in the specifications, indicated on the plans, and per the schedule. The work includes, but is not limited to, construction of an addition for MRI services along with demolition and renovation work adjacent to the addition for these services; demolition and renovation of the existing hospital areas for isolation rooms; generator replacement; associated site work; associated mechanical, plumbing, sprinkler, and electrical work. This project is partially funded with Emergency Rural Health Care funds and will be subject to Davis-Bacon and Related Acts requirements. QUESTIONS: Bidding questions are due no later than 4:00 PM CST on August 16th, 2023. Questions shall be submitted to the Architect and Construction Manager via email and send to representatives listed in the project directory. BID SECURITY: All bids must be accompanied by bid security payable to the Hospital in an amount equal to at least 5% of the highest total amount including Add Alternates. The bid security may be in the form of a Certified Check, Cashier's Check or a bid bond executed by corporations authorized to contract as surety in the State of Iowa. The bid bond shall be executed by the Bidder on RD Instruction 1942-A IA Guide 19 Attachment 4. Bid Security shall be in a separate envelope with the sealed bid and shall be forfeited to the Hospital as liquidated damages in the event the bidder fails to enter a contract and furnish a bond within ten days after his bid has been accepted. No Bidder may withdraw his bid for at least sixty (60) days after the scheduled closing time for the receipt of bids. The successful Bidder will be required to furnish and pay for satisfactory Performance Bond and Labor and Material Payment Bond in the amount of 100% of the Contract Price. The Bond must be issued by a responsible surety approved by the USDA- RD and Board of Trustees of the Hospital.
