SCOPE OF WORK Apex Environmental completed an asbestos and paint survey of the existing NE 42nd Avenue Bridge over NE Lombard Street in Portland, Oregon 1. Review of provided bridge plans for suspect Asbestos Containing Materials (ACMs) used in the NE 42nd Avenue Bridge construction. 2. Inspection of the NE 42nd Avenue Bridge by an Asbestos Hazards Emergency Response Act (AHERA) certified inspector for suspect ACMs. 3. Compiling a homogeneous building materials list for the NE 42nd Avenue Bridge. 4. Bulk sample collection of accessible suspect ACMs by an AHERA certified inspector. 5. Analysis of suspect ACMs sampled per the polarized light microscopy (PLM) method by a NVLAP certified laboratory. 6. Inspection of the NE 42nd Avenue Bridge for painted and coated surfaces. 7. Compiling a homogeneous list of painted and coated surfaces for the NE 42nd Avenue Bridge. 8. Collecting two samples for each accessible paint type observed on the NE 42nd Avenue Bridge. 9. Submittal of paint samples to the City of Portland Water Pollution Control Laboratory (WPCL) for analysis of total lead, cadmium, and chromium according to Method 6010 and polychlorinated biphenyls (PCBs) according to Method 8082. 10. Follow-up analysis by the Toxicity Leachate Procedure (TCLP) where the total lead, cadmium, or chromium concentrations exceed 20 times the toxicity characteristic leachate concentration per 40 CFR 261.24. 11. Preparation of this Asbestos & Paint Survey report detailing collection procedures, laboratory analytical results, a map showing sample locations, photographs of materials sampled, data tables summarizing laboratory results, copies of laboratory reports and sample chain-of custodies, and recommendations for materials handling. The 42nd Avenue Bridge Replacement project involves the construction of street, grading, drainage, paving, structures, signing, striping, illumination, water, and bridge improvements along NE 42nd Avenue from NE Holman St to NE Columbia St and NE Lombard Street from ODOT MP 8.22 to ODOT MP 8.33. BID SECURITY No bid will be considered unless accompanied by a certified check, cashier's check, or a bid bond for an amount not less than ten percent (10%) of the aggregate amount of the bid.
