This solicitation is being made available to NYS certified SDVOB, MBE, or WBE businesses at this time ONLY. A prospective contractor must hold one or more of these listed designations (SDVOB, MBE, WBE) to be considered for award of this solicitation. New York State Office of Temporary and Disability Assistance (herein also referred to as "NYS OTDA", or "OTDA") is seeking a qualified Contractor to provide HVAC planned preventive quarterly maintenance, and, an hourly rate for repairs. The term of the agreement is to be for a period of up to 2 years. 3 Facility locations, one each in: Albany, Buffalo, and Endicott. Contractor not required to bid on all Facilities. Award is to be made to a responsible contractor based on pricing per each facility, separately. Up to 3 separate awards may be issued. Bidders must include the following completed documents: -Bid form (see "1.RFQ-DD-HVAC..." attachment) -COI - Acord (see Appendix C for additional info) -Proof of Disability Insurance (see Appendix C for additional info) -Proof of Workers' Comp Insurance (see Appendix C for additional info) -Offeror Disclosure of Prior Non-Responsibility Determination (see attached) -Procurement Lobbying Act Offeror's Certification and Affirmation of Understanding of and Agreement pursuant to State Finance Law 139-j and k (see attached) Service-Disabled Veteran-Owned Set Aside: No Total MWBE Participation Goals: 100% Service-Disabled Veteran-Owned Business Contracting Goal: 100% Service-Disabled Veteran-Owned Set Aside: No Total MWBE Participation Goals: 100% Service-Disabled Veteran-Owned Business Contracting Goal: 100% Contract term: estimated up to 2 years
