Provision of Concrete, Installation, and Removal Services Question Deadline 08/12/2024 at 5:00 PM ET SPECIFICATIONS / SCOPE OF WORK PURPOSE 1. The purpose of this bid is to select a qualified "Flat Work Concrete Specialty Contractor" to provide all labor, materials, equipment and supervision necessary to perform the installation of concrete sidewalks, slabs, footers, driveways, driveway aprons, combination curb and gutters as well as other such types of "flat work" concrete installations at various facilities county wide, per specifications. The successful Contractor shall not subcontract to others for work or services performed during the term of this contract unless prior, written authorization from the Facilities Services Department has been obtained. When the services of a subcontractor(s) are required, prior approval of that subcontractor must be obtained from the Facilities Services Department. If the use of a sub-contractor is approved, it shall be the sole responsibility of the Contractor to regulate and guarantee work. The contractor is also responsible to pay subcontractors' expenses at no cost to the district. A proposed subcontractor form has been provided on page 28 of the bid. The Contractor is responsible for all payments, invoicing and documentation of their subcontractors. The SBSC will NOT be held responsible for ANY subcontractors' contract (i.e. payment, to subcontractor, awarding work to subcontractor, etc.). No purchase order payment will be issued to any firm other than the awarded Contractor. 2. Contractor Requirements A. The selected Contractor shall provide services as requested to district facilities, inside and outside, based upon the unit cost, hourly rates and percentage markup for discounted rates for parts and materials as stated in the bid. Under certain circumstances, parts and materials may be supplied by the district. Contractor shall supply all vehicles, equipment or tools necessary to perform all normal and customary tasks as related to the building trade. The district will not pay for rental or purchase of equipment needed by the contractor, nor will it pay for mobilization fees of any kind. In the event that the district supplies parts and materials for a specific project, the Contractor will be responsible for obtaining the parts or materials from the Facilities Services Department. B. The Contractor shall be a Licensed Flat Work Concrete Specialty Contractor licensed by the State of Florida, to perform the installation of concrete sidewalks, slabs, footers, driveways, driveway aprons, combination curb and gutters and other such types of "flat work" concrete installations. C. Contractor shall be able to produce a valid copy of its license(s) upon request. D. Contractor shall comply with all safety rules defined by State and Federal regulations and any other laws and regulations relating to the services being performed. E. Contractor shall construct and install concrete work activities as directed by the district, according to specifications and approved methods. F. When performing services as requested in this bid, Contractor shall provide: All labor,Supervision, Necessary Tools and Equipment, Trained Personnel, Materials, Parts, and Supplies if applicable G. Contractor shall not use personnel with suspended or revoked licenses. H. Contractor shall take direction only from the Project Coordinator of the Facilities Services Department or his designee. Contractor may be liable to pay for changes at its own expense if direction is taken from unauthorized personnel. I. Contractor shall supply the name of a contact person to include phone number and cell and/or beeper number who will receive work related calls from the Project Coordinator or his designee.
