OMV Interior Repairs - DPS Scope of Work - Leesville OMV: Contractor shall provide all labor and materials in order to complete the interior repairs of damages caused during Hurricane Laura and the Ice Storm of February 2021 to the Leesville Office of Motor Vehicles (OMV) for the Louisiana Department of Public Safety (DPS). Contractor shall: o Furnish all labor, equipment and supplies necessary for the completion of the stated repairs. o Move out, then reset all contents as required to complete project. o Disconnect/reconnect system wiring as needed in order to make repairs to building (mechanical room). o Mask and cover all surfaces and fixtures to protect from paint spillage, drippage, etc. o Coordinate work required after regular business hours (8:00 a.m. to 4:30 p.m.) with Ellen Tyree, office manager. Any work hour changes must also be coordinated and approved by office manager. o Provide job-site storage, with delivery and pick-up, as required to hold supplies and/or materials. o Dispose of all materials and debris resulting from the repairs and provide for removal of same. Deadline to receive written inquiries: 1/11/2024 Deadline to answer written inquiries: 1/18/2024 Consideration of bids: The Office of State Procurement reserves the right to reject any or all bids and in particular to reject a bid not accompanied by any required bid security or data required by the bid documents or a bid in any way incomplete or irregular. The Office of State Procurement reserves the right to waive any informality or irregularity in any bid received, deemed to be in the best interest of the State of Louisiana. Withdrawal of bids: The undersigned agrees that this bid shall be good and may not be withdrawn for a period of 30 calendar days after the bid opening. Bid security: IF THE BID IS $50,000 OR ABOVE, a bid security must be attached (insurance company, bank money order, certified check or cashier's check made payable to "State of Louisiana - Department of Public Safety") in the sum of 5% of the amount bid (including base bid and additive alternates, if any) and shall become the property of the Agency in the event the contract and bond are not executed within the time set forth above. a Performance Bond (made payable to "State of Louisiana - Department of Public Safety") with Power of Attorney in an amount equal to the contract sum (100% of the amount of the contract) and agrees that this bond will be secured by a surety or insurance company currently on the U.S. Department of the Treasury Financial Management Service list of approved bonding companies which is published annually in the Federal Register. In addition, the bond shall be written by a surety or insurance company that is currently licensed to do business in the state of Louisiana. Also, to be provided at the same time is a Labor and Materials Payment Bond (made payable to "State of Louisiana - Department of Public Safety") in an amount equal to 100% of the contract amount.
