Landstreet Road, Orlando, FL 32824 Project Description: Removal and installation of new concrete for construction of footers. Reinforced concrete foundation. New concrete infill for construction of footers. Provide signed and sealed delegated design for pre-engineered structure connections and standing seam metal canopy system and install. Provide signed and sealed delegated design for pre-engineered column attached to concrete pedestal (5' high) painted (safety yellow). Relocate and extend existing wall pack circuits to new locations and match existing voltage on new fixture. Provide new architectural wall pack BOD: HE Williams WPAL=L49/80-50-BZ-EM/6W/UNV. Existing wall mount conduit to remain, relocate as required to for canopy structure clearance. Existing overhead coiling doors to remain. Install fabricated seamless 6"x 6" metal gutters. Install fabricated 5"x 5" metal down spouts and precast concrete splash block. Contractor will be responsible for providing dumpsters and all items necessary for the removal and disposal of all construction materials. PREQUALIFICATION: Each bidder shall submit a current license issued by the State of Florida (Builders License or General Contractors License) and, if a Florida Corporation, a copy of the Corporate Charter as prequalification of their eligibility with the bid document to the Department of Transportation, Contracts Administration Office: 719 South Woodland Boulevard, DeLand FL 32720 BID BOND: If the bid amount exceeds $100,000, the bidder must provide with the bid, a Bid Guaranty of five percent (5%) of the actual total bid in the form of a certified check, cashier's check, treasurer's check, bank draft of any national or state bank, or a surety Bid Bond made payable to the Department of Transportation. A Bid Guaranty in an amount less than five percent (5%) of the actual bid will invalidate the bid. Bid Bonds shall conform to the Departments Bid/Proposal Bond Form furnished with the proposal package The bidder must submit an original proposal guaranty. A fax or copy sent as an attachment will not be accepted. PERFORMANCE BOND and LABOR AND MATERIALS PAYMENT BOND: If the contract award amount exceeds $100,000, both a Performance Bond and a Labor and Material Payment Bond of 100% each of the contract sum are required at the time of award.
