The Town of Enfield has been awarded Division of Water Infrastructure Wastewater State Reserve CWSRF Principal Forgiveness Funding in the amount of $7,351.01.00. Any awards pursuant to this procurement are contingent upon written receipt and approval of funding obligating documents. No funds will be obligated until appropriate documents are executed, and conditions removed Address any questions to: Mr. Joe Dooley, Grant Consultant (252) 258-2497 This RFQ format has been prepared on behalf of the Town of Enfield at the request of the Town. The RFQ contents have been reviewed in detail and approved for use in this program. Small, womens business enterprises and/or minority owned firms are encouraged to submit qualifications The proposed project consists of the following activities. Town of Enfield Phase 6 Sewer System Improvements Project Scope of Requested Services 1. Necessary engineering surveys (not to include property or right-of-way surveys) 2. Preliminary design sketches, estimates and documents, plans, specifications, and contract documents necessary for the work 3. Preparation of specifications and contract documents for any necessary laboratory testing proposals 4. Administration and letting of all construction related contracts 5. Assistance in making contract awards execution 6. Conduct Prebid and allied Preconstruction Conferences, with appropriate minutes 7. Weekly site visits to observe progress and quality of the work 8. Determination of whether the work is in accordance with the NC DEQ funding, documents, plans and specifications 9. Shop drawing review 10. Processing of construction contract change orders (not for changes substantially changing the scope of the project) 11. Review and processing of contractors application for payment, including compliance with Davis Bacon and American Iron and Steel provisions (if applicable) 12. Final inspections to ensure compliance with the contract documents, plans and specifications 13. Written approval of final payments to contractors 14. Furnishing two sets of blue line as-built drawings 15. Construction administration/observation 16. All other engineering/inspection services necessary for project completion 17. Compliance with Build America Buy America Act Proposal Contents 1. Name of firm 2. Physical location of office(s) (street and mailing address) 3. Brief history 4. Firm employment profile 5. Prior Division of Water Infrastructure successful engineering/construction administration/ construction observation experience 6. General engineering/construction administration/construction observation experience 7. Personnel involved with active execution of the engineering/construction administration/ construction observation activities. Include a brief description of the proposed responsibilities of each with a brief resume. 7. Brief management plan as same pertains to municipal sewer related work 8. Hourly rate schedule (to include all travel, overhead and miscellaneous expenses.) In accordance with NCGS 143-64:DO NOT INCLUDE a lump sum price for services. 9. SAM.GOV Search and reprint and NCDEA State Deb Vendor List Evaluation Criteria I. The Town shall conduct the procurement in accordance with 24 CFR 85.36 (Competitive Negotiation). Qualifications received by the deadline and prepared in accordance with the RFQ instructions will be reviewed to determine the most advantageous level of service.2. The Town will select the Consultant best qualified to provide the desired level of service and address the long-term best interest of the Town without regard to fee other than unit price information. 3. After selection of the best qualified firm to meet the criteria above, the Town shall negotiate a fair and reasonable fee with the firm for the desired level of service. 4. The Town will evaluate the qualifications using the following criteria. 1. General qualifications, competence, and reputation...(20 points) Age, size, employment profile and stability of the firm inclusive of professional and non-professional staff Identification of Management and Staff assignments NC Licensure Sub-consultants if any 2. Firms capability to meet time and project budget requirements of NC DEQ for Phase 6 Sewer System Improvements (20 points) Program including but not limited to milestones and other contractual constraints depicted in contract between the State and the Town Understanding and execution of program requirements Management and design philosophies Ability to maximize local benefit through program design. Hourly Rate Schedule Do not include a lump sum or not to exceed price. 3. Present and project workload of firm depicting the firms ability to understand and devote necessary quality time and effort to the subject project (20 points) Projects successfully completed demonstrating qualifications and competence for the successful, timely completion of the project. Ability to provide access to qualified project team members on a continual basis. Program knowledge and flexibility to address local needs. 4. Related experience on projects similar to Phase 6 Sewer System Improvements Projects (20 points) Volume of DWI engineering projects completed successfully and timely without multiple contract extensions. Reputation with previous clients Experience with similar type improvements Grasp of project requirements Ability to implement project requirements. Design approach/ methodology/Cost effective design 5. Past and recent work for the entity issuing RFQ (20 points) Demonstrate understanding of area, inclusive of soil conditions, etc. Knowledge of system to be worked on and understanding of system needs Familiarity of projects geographic area Proposed project management plan The method of Contractor Selection has not been Determined at this time.
