The Contractor shall contact Deputy COA Clerk Leanne Williams, Custodian of COA Public Records, at (386) 418-6104 or lwilliams@cityofalachua.com, concerning any questions the Contractor may have regarding the duty of Contractor to provide Public Records. The City of Alachua owns and maintains lime rock roadway within the corporate city limits. Approximately 10,737 linear feet (~2.3 miles) of lime rock roadways will be contracted for maintenance. The attached map identifies these locations within the corporate city limits. Each lime rock roadway is graded on a monthly basis to maintain roadway integrity and minimize pothole formations. MATERIALS All lime rock materials shall be in accordance with FDOT specifications. Materials shall be added during the grading process as needed. Supervisor or designated representative authorization will be required prior to installing additional lime rock to the roadway. MEANS & METHODS Grading shall be performed by a prequalified grading contractor with a minimum of (5) year's of experience in this type of work. Work shall be completed by the 15th of each month with invoices submitted at the end of the month for all work completed. Invoices will include the location, number of hours worked, and number of loads of lime rock installed. Unit pricing will be included with the extended costs shown for each section of roadway graded. Insurance Contractor shall provide the City of Alachua Commercial General Liability Insurance in the amount of $1,000,000.00 with the City of Alachua named as "Additional Insured". Work Scope Includes: Mobilize to job site. Obtain utility locates. (As may be required) Exercise care when working in close proximity to existing utility facilities. Landscape disturbed areas adjacent to construction to previous condition. Facilitate Maintenance of Traffic. Provide materials, labor and equipment to perform the work. Demobilize from jobsite to jobsite as required.
