Weber Housing Authority (Owner) and it's hired design professional, MSR Design (MSR) are interested in working with a qualified Construction Management/General Contractor (CMGC) Firm ("Proposer") and requests proposals for construction management services, for the pre-construction phase, and general contractor services, for the construction phase, of an extensive interior remodel of Aspen Assisted Living, located in Ogden, Utah ("Project"). Aspen Assisted Living is located at 2325 Madison Avenue, in Ogden, Utah. The building is comprised of 21,952 square feet on a 1.0-acre lot. The facility was constructed in 1963 with a full remodel in 1991. In 2013 the facility was converted from a skilled nursing facility into a Level II assisted living facility. Another extensive remodel was completed in 2018. A crawl space provides access to the entire facility for easy access to utilities throughout. The facility is comprised of two 16-unit "L-shaped" wings, surrounding a central outdoor courtyard. The units vary in size and configuration. Each unit has a jack-and-Jill bathroom design. The facility holds a full commercial kitchen, a reception area, a dining room and family room, laundry facilities, multiple offices, and plenty of on-site storage. By submitting a proposal in response to this RFP, offeror is acknowledging that the requirements, scope of work, and the evaluation process, outlined in the RFP are fair, equitable, not unduly restrictive, understood and agreed to.
