The Senior Center Project The Scope of Work consists of: To furnish and install all labor, material, tools, and other items to complete this work to include, but not limited to the following Remodeling 1. Demolition and patching at existing wall conditions 2. Framing of new walls with steel studs 3. Drywall hanging and taping 4. Electrical demolition and installation of new openings 5. Duct modification-only as indicated on the plans 6. No plumbing included 7. Re-use and relocate interior glass windows and doors from office and re-use as shown 8. Take one existing interior door from an interior (unseen) room in building and install it at new office then take the new pre-stained slab and put it in the unseen room 9. New bypass doors for office closets to pre-stained or stained on-site to match existing doors as close as possible. 10. All cove base and flooring demolition and reinstallation by others 11. All painting by others 12. Owners shall provide a dumpster Flooring 1. Remove and dispose of carpeting as indicated on the plans 2. Float and patch floor areas at uneven spots in existing tile areas 3. Infill areas at former carpet with new VCT to ensure floor planes out properly 4. Use a high PH stripper as manufacturer's instructions by letting the liquid soak in and then using a 175 RPM scrubber. Ensure all residue is removed and rinsed and cleaned with a PH neutral cleaner. Once complete and dry, test an area to ensure a st rong adhesive bond. Install new LVP and base pe r manufactu re r 's specification. 5. Install glue down Patc raft LVP pe r manufactu re r's specifications 6. Demolish existing VCT in kitchen, prep floor as required and install new porcelain floor and rubber base 7. Remove all vinyl base at areas to receive new flooring by cutting the drywall paper immediately above to base and install new 6" Patcraft rubber cove base 8. Moving of all furniture and equipment by owner in phases (sections). Painting 1. Base board will be removed at locations as indicated on the drawings 2. Patch and sand all holes and cracks as required including areas of former walls 3. Prime at all patches and new drywall at new offices 4. Install two coats of Sherwin Williams ProMar 200 Eggshell paint at all wall locations indicated on Sheet number 4, in the plans, on the Room Finish Schedule 5. Install two coats of Sherwin Williams 200 Flat paint at all soffits in areas where wall painting will be done 6. Paint new office bypass doors semi-gloss white 7. Moving of all furniture and equipment by owner in phases (sections). All submitted bids shall contain an Indiana Form 96 including, without limitation, financial statement, non-collusion affidavit and certification of non-segregated facilities of the bidder. In accordance with IC 36-1-12-4.5, each bid in excess of Two Hundred Thousand and No/100 Dollars ($200,000.00) shall be submitted with a certified check or bidder's bond in an amount of not less than Ten Percent of the bid amount as a guarantee that the bidder will perform the terms of the bid if it is accepted by the Parks Department. In accordance with IC 36-1-12-13.1, in the event that the successful bid exceeds the sum of Two Hundred Thousand and No/100 Dollars ($200,000.00), the successful bidder will be required to submit a payment bond. In accordance with Michigan City Ordinance No. 4351, each bidder shall be required to comply with the Michigan City Equal Opportunity Program in order for the Parks Department to consider its bid. Each bidder must submit a written affirmative action program with a bid, which program shall include specific goals and timetables for the employment and training of minorities and of lower income residents of the project area. Each bidder must ensure that all employees and applicants for employment are not discriminated against because of race, creed, color, sex, handicap or national origin Each bidder must submit an Affidavit of E-Verify Compliance and Certification of Non-Investment in Iran. If the successful bid exceeds the sum of One Hundred Fifty Thousand and NO/100 Dollars ($150,000.00), the successful bidder will be required to comply with Michigan City Hiring The Parks Department reserves the right to waive any formality, informality, or irregularity in any bid or proposal received, and also reserves the right to reject any and all bids and proposals and to re-advertise for bids or proposals.
