SINGLE-OVERALL CONTRACT BID Sealed Bid Proposals shall be received as follows: The bidder shall be classified by the New Jersey Department of the Treasury, Division of Property Management and Construction (DPMC) in the following trade(s): C008 GENERAL CONSTRUCTION or C009 GENERAL CONSTRUCTION/ALTERATIONS AND ADDITIONS or C032 HVACR Subcontractors for each of the major branches of work specified in N.J.S.A. 18A:18A-18 and/or named in the Form of Bid Proposal for Plumbing, Heating Ventilating and Air Conditioning, and/or Electric, who perform any work on the Project must be prequalified prior to the submission of bids, pursuant to the State of New Jersey Division of Property Management and Construction (DPMC). Bidders shall submit a Certificate of Prequalification/Classification from the New Jersey Department of Treasury, Division of Property Management and Construction (DPMC) and Affidavit as to the Total Amount of Uncompleted Contracts for the bidder and all major subcontractors. Bids must be accompanied by bid security in the form of (1) a bid bond or certified or cashier's check payable to Perth Amboy Board of Education equal to 10% of the amount of the bid, but in no case to exceed $20,000.00 as per N.J.S.A. 18A:18A-24 and (2) Certificate of Consent of Surety as per N.J.S.A. 18A:18A-25. The Owner requires that all Bids shall comply with the bidding requirements specified in the INSTRUCTIONS TO BIDDERS. Bidders are required to comply with requirements of N.J.S.A. 10:2-1 et seq., N.J.S.A. 10:5-1 et seq., "The Law Against Discrimination," and affirmative action, N.J.S.A. 10:5-31 et seq. and N.J.AC. 17:27-1 et seq. concerning equal employment opportunity and affirmative action and Chapter 33, Public Laws of 1977 (N.J.S.A. 52:25-24.2) requiring submission of an Ownership Disclosure Statement. All Bidders are required to complete and submit a Disclosure of Investment Activities in Iran Form in accordance with N.J.S.A 52:32-58 and N.J.S.A. 18A:18A-49.4. All Bidders are required to submit an affidavit by the Bidder that the Bidder is not, at the time of the bid, included on the New Jersey State Treasurer's List or the Federal Government List of Debarred, Suspended or Disqualified Firms or individuals pursuant to N.J.A.C. 17:19-4 and N.J.A.C. 19:38A-4. All bidders must comply with the requirements of the Prevailing Wage Rate Determination pursuant to N.J.S.A. 34:11-56.25. All Bidders must comply with N.J.S.A. 18A:18A-20 "American Goods and Products to be used where possible." The Contract, if awarded, shall be awarded to the lowest responsible bidder as defined in N.J.S.A. 18A:18A-2 whose bid is responsive in all material respects to the bid requirements.
