Electrical Maintenance, Repair and Response. Location: Long Island DDSO This is an RFQ (request for quotes). This is not an IFB (bid). The Long Island DDSO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work The Long Island DDSO contact for this project is: John Goldschmidt,516-576-5026, john.s.goldschmidt@opwdd.ny.gov Contract term: 12 Months with the right to terminate earlier Long Island DDSO reserves the right to reject any and all quotes. In deciding whether to or not to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker's Compensation Insurance as well as completed Lobbying Law Forms, and Signed Appendix A. Please be prepared to submit these in a timely manner. The Long Island DDSO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition. The Long Island DDSO 's interpretation of specifications shall be final and binding. Please note all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitors log and/or an attendance sheet provided by the maintenance department. Electrical Maintenance, Repair, and Response. General requirements This is a maintenance and emergency services contract. For the purpose of this contract, the contractor's bid price shall include, but is not necessarily limited to all tools, equipment and travel costs to perform the service. This contract is on a time and materials basis. Contractor shall have a minimum of five (5) years in a service and repair experience of electrical equipment in residential building and commercial building. Contractor is required to submit a list of five (5) references; at least one reference shall be from a healthcare facility. Contractor must provide copies of all licenses, certification, and references to the Business Office for processing a formal bid. Contractor must attend a kickoff meeting with the Plant Superintendent before starting the contract. This meeting will be scheduled after the contractor is awarded an approved contract. Work schedule is Monday through Saturday, except NY state holidays, from 8 a.m. to 5 p.m. except with prior written approval. Contractor shall provide 7 days and 24 hours emergency services and have a four (4) hour maximum response time. Contractor shall provide NYS licensed electrician to perform all works. All work shall be complied with latest NFPA 70 and/or NEC Code and executed in accordance with the best accepted trade practices. Contractor must provide all labor, tools, and equipment necessary to professionally execute the work. All material and equipment furnished shall be new and in excellent working condition. Contractor must use products and brands according to manufacturer's specifications or must get prior written approval from Plant Superintendent's office. Contractor shall clean up all areas and remove all debris and waste from the facility and properly dispose of said debris and waste. Contractor must take all precautions to leave the work area as safe and hazard free at the end for each day. Use of drug, use of alcoholic beverages, or smoking by the Contractor or its personnel shall not be permitted on the facility.
