For Requirements Contract for Concrete Curbs Side-walks and Aprons Each bid must be accompanied by either a certified check on a solvent hank or trust company acceptable to the Town, or a bid bond from a surety prepared in such a form as is acceptable by the Town. The certified check or bid bond shall be in the amount of five percent 15%) of the total hid price and be made payable to the Town of Smithtown as assurance that the contract will be executed if awarded to such bidders. The Town of Smithtown reserves the right to reject any and all bids. DESCRIPTION: The work under this Item shall consist of the clearing, grubbing, trimming, grading, removing and disposing of all trees, brush, stumps, topsoil, vegetation, debris and other miscellaneous structures not covered under other contract items within the construction and in a workmanlike manner to the lines and grades as shown on the plans or as directed by the Superintendent. All excess materials from the Contract are the property of the Town of Smithtown and such material shall be legally disposed of by the Contractor as ordered by the Superintendent at no additional cost to the Town. Under this Item the Contractor shall furnish and place selected materials in accordance with the requirements of Section 203, Excavation and Embankment, of the New York State Department of Transportation Standard Specifications as it relates to Select Granular Subgrade, except as modified herein, and as shown on the plans or as ordered by the Superintendent. This work shall consist of furnishing, installing, inspecting, maintaining, and removing soil erosion and sediment control measures as ordered by the Town Engineer during the life of the contract. Under this Item the Contractor shall furnish and place a dense graded aggregate subbase course of material to the lines, grades, thicknesses, and typical sections called for on the plans or as necessary to change the gradation of an existing material all in accordance with Section 304, Subbase Course, of the New York State Department of Transportation Standard Specifications. The contractor is responsible for Quality Control (QC). QC is defined as all activities required to produce hot mix asphalt (HMA) that meets all specification requirements. The contractor shall incorporate a Quality Control system for all plant production of HMA and assume responsibilities for all QC activities at the production facilities. The asphalt production shall be currently approved to produce HMA for the New York State Department ofTransportation (scales and automation). The Town is responsible for Quality Assurance (QA). QA is defined as all activities performed by the Town to assure that HMA production meets the specification requirements.
