Lake Shore Drive Bikeway Improvements The approximate quantities of major work items are as follows: DENSE GRADED AGGREGATE, 4" TH. 5,200 SY H.M.A. SURFACE COURSE 750 TONS H.M.A. MILLING 1,030 S.Y. All bids must be submitted in the form of proposal prescribed by the Borough of Haworth.All bids must be submitted in the form of proposal prescribed by the Borough of Haworth. All proposals must be accompanied by a bid bond or certified check to the order of the Borough in an amount not less than ten percent (10%) of the bid or $20,000.00, whichever is less, for each bid submitted. Bidders are required to comply with the requirements of P.L. 1977, c. 33, Laws of 1977 regarding disclosure of Partners and Stockholders, and P.L. 1975, c. 127 regarding Affirmative Action, and Executive Order 11246 regarding Equal Opportunity. Bidders must furnish a list of subcontractors to be used on the project. This bid solicitation is made in a fair and open process in accordance with N.J.S.A. 19:44A-20.5 et. seq. Pursuant to P.L. 2004 c.57, all bids must be accompanied by a New Jersey Business Registration Certificate issued by the New Jersey Department of Treasury, Division of Revenue. Failure to include a said certificate is a material bid defect which cannot be waived. The proposal shall also contain consent of surety agreement under which the surety company agrees to provide a performance and payment bond if the contract is awarded to its principal. All bids shall be irrevocable and shall not be subject to withdrawal for any reason for a period of sixty (60) days following the date fixed for the opening thereof . The Borough of Haworth reserves the right to waive immaterial informalities in any bid or to reject all bids if the best interest of the Borough so requires. This project includes the following work tasks: 1. Installation of all temporary traffic controls to close and protect the work zone. The contractor shall provide all required traffic controls to protect the public and the work operations in accordance with the Work Zone requirements of the current Federal "Manual of Uniform Traffic Control Devices." 2. Excavation and preparation of existing subgrade for bikeway. 3. Milling of existing asphalt on portions of the bikeway. 4. Installation of 4" th. virgin dense graded aggregate along bikeway. 5. Paving of bikeway. 6. Installation of topsoil, seed and straw mulch. 7. Installation of regulatory and warning signs and pavement markings. 8. Restoration of site. 9. Clean up the work zones to the satisfaction of the Engineer and Borough
