Anderson School District Five would like to contract with a vendor to Provide and Install a 3' x 3' "Hollywood Star" in the Auditorium Floor at Southwood. See specifications below for details. The successful contractor shall assume all responsibilities associated with this contract including but not limited to all labor fees, federal, state, city or town required business license(s), permits, insurance, fuel, supplies and any other expenses required to carry out the requirement of this solicitation. The successful contractor shall be properly licensed (Section 40-11-30) and must have at least five (5) years of documented experience. The award will be to one (1) contractor. SPECIFICATIONS Location: Southwood Academy of the Arts 1. See example on page 4. 2. Contractor must provide a mock up with design, colors, materials, measurements, etc. 3. Hollywood Star should consist of Terrazzo with Brass Inlay for Star, Letters & Emblem 4. Contractor will be responsible for all base work, installation & clean up during & final clean up after installation. 5. Site visit is available upon request 6. All debris will be removed and all areas cleaned to original stat before final payments are submitted 7. Before leaving each day, site must be cleaned up and all trash disposed of off-site 8. All debris must be removed and disposed of off-site 9. All meetings require a prior notice of 24 business hours 10. No tobacco products of any kind on school grounds 24/7/365 11.
