1. The proposed work consists of two phases of construction on Agner Street. Phase 1 (OPWC Rd. 37) from 4 th Street to 11th Street includes approximately 7667 SY of Pavement Planning, 426 CY of Asphalt Concrete, 661 LF of Curb and Gutter replacement, 937 SF of Driveway Approach Replacement, Adjust 6 Manholes, Water Valve and 9 Catch Basins to grade. The engineers' estimate for Phase #1 is $195,000. 2. Phase 2 (OPWC Rd. 38) from 4th Street to Main Street includes approximately 2,558 SY of Pavement Planning, 227 CY of Asphalt Concrete, 882 LF of concrete curb replacement, 2,336 SF of concrete sidewalks, 1,181 SF of Driveway Approach Replacements, Adjust 5 Manholes, Water Valves and 3 Catch Basins to grade. The engineers' estimate for Phase #2 is $140,000. Total Estimate of Phase 1 & 2 is $335,000. Bids must be submitted on the unbound Bid Forms contained in the Bidding Documents and must be accompanied by a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code or Bid Security furnished in Bid Bond Form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. This project is partially funded by the OPWC Infrastructure Grant & Loan Programs (Rounds 37 & 38). Attention must be given to all requirements contained in the bid packet, prevailing wage rates, various insurance requirements and the requirement for a payment and performance bond for 100% of the contract price. Bidders must comply with the Prevailing Wage Rates on Public Improvements in Putnam County, Ohio as determined by the Ohio Bureau of Employment Services, Wage and Hour Division. All questions about the meaning or intent of the Bidding Documents are to be submitted to Engineer in writing. Interpretations or clarifications considered necessary by Engineer in response to such questions will be issued by Addenda mailed or delivered to all parties recorded by Engineer as having received the Bidding Documents. Questions received less than ten days prior to the date for opening of Bids may not be answered. Only questions answered by Addenda will be binding. Bids will only be accepted from the Bidders listed on the Engineer's Official plan holders list.
