Asbestos Abatement& Demolition at 103 Debaillon St Contractors or contracting firms submitting quotes shall certify that they are licensed under Chapter 24 of Title 37 of the Louisiana Revised Statues of 1950 and show their license number on the outside of the sealed envelope in which their quote is enclosed. Quotes are to have the license number and the Contracting Firm/Company Name on the outside of the envelope. Contractors must be licensed for the classification of "Asbestos Abatement". Work must be completed within two (2) weeks of notice to proceed given by the Compliance Division. The successful contractor must not begin work until a notice to proceed has been issued by the Project Supervisor. All federal, state, parish, and city taxes due and payable on this bid shall be included in the amount bid on the quote form. The successful vendor shall be required to furnish the attached certificate of Insurance Liability that includes the following, as stated on the Information Purposes Only sample provided with the bid package: Item I-Workers' Compensation with Statutory Limits and Employers; Liability of $1,000,000; Item II - Commercial General Liability with minimum coverage of $500,000 to include Pollution Liability, Asbestos Abatement Liability, Underground Explosion and Collapse Hazard (XCU) in the amount of $500,000, and Item III Business Automobile Liability of $500,000; Item V- Lafayette City-Parish Consolidated Government, its officials, employees, and volunteers' as additional insureds; Item VII A waiver of subrogation provided in favor of the named additional insureds on the Workers' Compensation policy.
