Terms: All invoices and statements are to be mailed to the Yale Public School District, Accounts Payable Department, 198 School Drive, Yale, MI 48097 As required by State Law (PA 232 of 2004), all proposals/bids must be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the Owner or employee of the bidder and any member of the Yale Public School District Board of Education member or its superintendent. A certificate of insurance naming the Yale Public School District as an additional insured for commercial general liability insurance must be on file in the Business Office before any work is performed. We also require proof of Worker's Compensation coverage with Yale Public School District named as a certificate holder. The required policies & certificates are to be endorsed to give the district at least 30 days' notice of cancellation or material reduction of insurance coverage. The preferred General Liability insurance limit is $1,000,000. If you do not have this limit, please send a letter of explanation detailing the reasons you feel your limits are appropriate.
