The proposal shall be submitted with the attached Bid Cover Sheet completed in its entirety. Warranty information, when applicable, shall also be included & submitted. All work to be performed during normal working hours. If bid price is over $25,000 or more, bidder must be signatory to relevant union, per terms of the Various Trades Contract and Project Labor Agreement. If bid price is over $75,000, you MUST include your MBE/WBE subs and relevant forms at time of bid. Provide Labor and Materials: LBP Mitigation ACM Abatement 1) LBP mitigation in 3rd Floor Boy's Bathroom, 3rd Floor Girl's Bathroom, 2nd Floor Girl's Bathroom and associated Vestibule, 2nd Floor Boy's Bathroom, Room 213, 1st Floor Boys Bathroom, 1st Floor Girl's Bathroom, Room 109 Clinic, Social Work Office, Room 109B Office, Principal's Bathroom, Room 108, Engineer's Office, Engineer's Office Bathroom, Custodian's office, Custodian's Bathroom. ACM abatement in Main Office Storage Rooms and 1st Floor Center Corridor 2) Contractor to field verify dimensions and specifics of all projects listed above before placing bid. 3) Not responsible for contractor errors as a result of non-field verification. 4) Materials and installation must comply with CPS Standards and Specifications as listed on a) Section 01 56 11: General Dust, Fume and Odor Control b) Section 02 86 13: Hazardous and Universal Waste Management 5) Secure job site overnight if work is not completed in one day. 6) If building or CPS properties are damaged during construction, contractor to replace and/or repair as necessary. 7) Provide necessary labor and materials to fix/correct issues preventing any or all of the projects to be marked as completed. 8) All work shall be coordinated with the Board authorized representatives to complete in an agreed upon time frame. 9) Contractor is to remove and dispose of materials torn out and any construction debris. 10) All trash must be removed off-site by contractor. 11) Clean the surrounding area and make sure all is swept of debris. 12) All questions/requests must be received two (2) business days prior to bid closing.
