LICENSING REQUIREMENTS The AAC and all personnel associated with this project must be properly trained and have a current license or registration with the Texas Department of State Health Services for asbestos related activities for which they are to perform. Proof of licensing or registration is to be presented to the AC for review prior to the start of work. This will include a certificate of insurance naming the Owner as additional insured.] This requirement will apply to all subcontractors as well. SCOPE OF WORK Base Work ACBM Items to be removed: 1) 1st Floor: Remove floor tile and ACM adhesive from the two restrooms and a break room. There is approximately 381 SF of floor space. The removal of non-acm base molding will be required. 2) 2nd Floor: Remove floor tile and ACM adhesive from the Front Lobby and Closet. There is approximately 359 SF of floor space. The removal of nonacm carpet and base molding will be required. NON-ACBM WORK 3) Remove all floor tile and base molding from two rooms. Please refer to the attached drawing. 4) Clean / Remove all visible mold. Remove all ceiling tiles and other porous materials. Sanitize the space. All work will be conducted as per the Mold Protocol included in this set of documents.
