This project provides for the furnishing of all labor, material, equipment and work necessary for the replacement of a single span steel plate girder bridge with a two-span prestressed concrete spread box beam bridge that carries S.R. 1001 over the Northeast Extension of the Turnpike. Work also includes reconstruction of the S.R. 1001 approach roadway and Turnpike shoulders, maintenance and protection of Turnpike and local roadway traffic, and drainage improvements. Work items include, but are not limited to: 7,054 CY Excavation, Various Classes 674 CY Structure Backfill, Coarse Aggregate No. 57 286 TON Superpave Asphalt Mixture Design, Base Course, Various Depths 1,096 TON Superpave Asphalt Mixture Design, Binder Course, Various Depths 1,185 TON Superpave Asphalt Mixture Design, Wearing Course, Various Depths 665 TON Subbase (No. 2A) 1,404 LF Concrete Barrier, Various Types 2,889 LF Temporary Concrete Barrier, Various Types 106 CY Class AA Cement Concrete 352 CY Class A Cement Concrete 190 CY Class AAAP Cement Concrete 100,636 LB Reinforcement Bars, Epoxy Coated 751 LF Prestressed Concrete Spread Box Beams Milepost A-83.88 is located approximately 2.9 miles south of the Route 903 Interchange, Exit 87, in Penn Forest Township, Carbon County, Pennsylvania A Prequalification Certification and Applicable Capacity Rating assigned by the Prequalification Office of the Pennsylvania Department of Transportation is a necessary prerequisite for bidding on this project. An electronic bid bond, from an approved vendor, with corporate surety, in the amount of five (5%) percent of the bid must be submitted with the proposal as bid security. The bid security of the three (3) low bidders will be retained until the execution of the contract. (The normal time for execution of the contract is approximately four [4] weeks.) The Provisions of the Pennsylvania Prevailing Wage Act of August 15, 1961, P.L. 987, as amended, together with the rates and regulations promulgated by the Secretary of Labor and Industry applicable thereto, shall apply to this contract. The opening and listing of bids not meeting all the terms and conditions of the bid advertisement or bid procedures does not act as a waiver or modification of any requirements contained in the bid advertisement or bid procedure.
