Labor, Materials and Equipment Necessary to Install an Outdoor Fitness Station at Williams Playlot The Parish of Jefferson reserves the right to cancel this contract for convenience by issuing a thirty (30) day written notice to contractor. We extend this bid to cover all labor, materials, equipment, and necessary essentials to install ONE (1) new fitness station, which includes the equipment, slab, and safety surface at Williams Playlot, 7744 Mistletoe St., Metairie, LA 70003. Purchases for this project shall be exempt from state sales and use tax according to La.R.S. 47:301(8)(c)(i). The successful bidder shall be granted the tax-exempt status of Jefferson Parish via Form R-1020, Designation of Construction Contractor as Agent of a Governmental Entity Sale Tax Exemption Certificate. Form R-1020 is distributed by the Louisiana Department of Revenue. The successful bidder shall supply all labor, materials, equipment and necessary essentials to perform the following at the site mentioned above. o Location of the slabs will be determined by the owner. o Install 27' x 48' x 8'' thick with 3000 PSI and fiber mesh reinforcement concrete slab. Concrete footers beneath each event structure, as required, shall be include under slab so area has a level broom finish. o Sidewalks shall be 5' (foot) Wide with 30' (foot) Long with same specifications as above. o Pour-in-Place rubber surface in specs for fitness stations, color shall be picked from color chart provided by awarded vendor. o All colors to be selected by owner from color chart provided by awarded vendor. o Parks and Recreation will not be responsible for any materials or equipment on site while job is in progress. o Awarded vendor must provide their own dumpster on site to dispose of debris and must get dumpster location approved. o Vendor shall be responsible for furnishing and installing all equipment, parts, supplies, supervision, and personnel needed, plus any permits, fees, etc. needed to complete this job. o All work to be done in workmanlike manner. o Contractor shall be responsible for any damage to grounds, concrete and structures, etc. Contractor shall take pictures and video before starting job. o Specifications for Greenfields Outdoor Fitness - Small Sample Package & Rubber Surfacing, per below specifications - or approved equal.
