WELL NO. 3 IMPROVEMENT PROJECT - PHASE III in accordance with the Plans and Specifications prepared by Stephen Hovancsek & Associates, Inc. (the "Engineer"), on file for review and available for purchase in its offices, Two Merit Drive, Richmond Heights, Ohio 44143. CONTRACT WELL NO. 3 IMPROVEMENTS - PHASE III ESTIMATE $1,250,000.00 All questions should be directed to the Village Engineer, Thomas Cappello, P.E., P.S.., at Stephen Hovancsek & Associates, Inc., Two Merit Drive, Richmond Heights, Ohio 44143, phone (216) 731-6255 or (216) 731- 7152, fax (216) 731-4483. Email: tcappello@hovancsek.com Any Proposed Equal for a Standard shall be submitted to the Engineer not later than 7 days prior to the bid opening. If no Addendum is issued accepting the Proposed Equal as an Approved Equal, the Proposed Equal shall be considered rejected. Each Bidder must be a Bidding Document Holder registered with Stephen Hovancsek & Associates, Inc. The Village of Middlefield, Ohio anticipates receiving funding form the Water Supply Revolving Loan Account(WSRLA) and from the Ohio Public Works Commission (OPWC) to aid in financing the Project. The requirements for Bidders, Contractors and Subcontractors are explained in the Bidding Documents. The Village of Middlefield anticipates awarding the contract and providing a Notice to Proceed within Ninety (90) days of the Bid Opening. The contractor shall take this into consideration during bidding and shall hold all prices accordingly to the substantial completion of the project which is Three Hundred (300) Days from the Notice to Proceed.
