Please submit quote for individual line items. This solicitation is for all or none. All quotes shall be FOB destination. Include any discounts that may apply. The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable WARRANTY INFORMATION IS REQUIRED IN RESPONSE TO THE RFQ Description of Service: The contractor shall provide a comprehensive maintenance program for the 673d Medical Group Dining Facility, Joint Base Elmendorf-Richardson, Alaska. The maintenance program should ensure smooth operations and longevity of our kitchen appliances, minimizing downtime and maximizing efficiency. Scheduled Maintenance. The contractor will ensure optimum performance, safe operation, minimum downtime, and the maximum useful life of equipment. They will provide regular and systematic servicing, verification of performance and safety, and detection and replacement of worn or failing components before a serious problem develops. Preventative Maintenance. The contractor will provide the systematic care, servicing, and inspection of equipment quarterly for the purpose of maintaining it in a safe and serviceable condition and detecting and correcting all minor faults before they develop into major defects. Calibration/certification. The contractor will execute the measurement and adjustment of various equipment parameters as needed to ensure the accuracy is within prescribed standards. Unscheduled Maintenance. The contractor will troubleshoot to isolate the cause of equipment malfunction and replace or adjust components or subsystems to restore normal function, safety, performance, and reliability of medical equipment. If the unscheduled maintenance is an emergency the contractor will report within 2 hours of notification and if it is remedial, they will report within 24 hours of notification. Nutritional Medicine will make the determination if required maintenance is an emergency or remedial based on inpatient mission needs. Period of Performance: Base Year: 15 Sep 2023 - 14 Sep 2024 Option Year 1: 15 Sep 2024 - 14 Sep 2025 Option Year 2: 15 Sep 2025 - 14 Sep 2026 Option Year 3: 15 Sep 2026 - 14 Sep 2027 Option Year 4: 15 Sep 2027 - 14 Sep 2028 This solicitation will incorporate a site visit to allow any interested vendors the ability to see the building layout. The site visit will be on Friday, 4 August 2023 at 10:00 AM Alaska Daylight Time (AKDT). All vendors shall plan to meet at the Joint Base Elmendorf-Richardson Boniface Visitor Center no later than (NLT) 9:40 AM AKDT on 4 August 2023. If you are going to attend the site visit, please fill out the information on the "Site Visit Vendor Information" document attached. Every contractor who wishes to attend the site visit must provide their information. Please return the attached "Site Visit Vendor Information" document to the POC's NLT Wednesday, 2 August 2023 by 4:00 PM AKDT.
