All bidders must file a "Bidders Proof of Responsibility Form" with the Village Engineer on or before 4:00 P.M. on August 17, 2023. \ A Certified check or bid bond payable without condition to the Village of West Milwaukee in an amount not less than five percent (5%) of the bid shall be submitted with each bid as a guarantee that if the bid is accepted, a proper contract and bond will be executed and filed within ten (10) days after the acceptance of the bid. If such bidder fails to execute and file such contract and bond, the amount of the check or bid bond shall be forfeited as liquidated damages. No bid shall be withdrawn after the opening of bids without the consent of the Village of West Milwaukee for a period of sixty (60) days after the time of opening bids. Each bidder agrees upon submission of bid that if the same be accepted within said time period, he shall be bound by the terms of acceptance hereinafter contained. The Village of West Milwaukee reserves the right to reject any or all bids received or to accept any bid which may be to the best interest of the Village of West Milwaukee. Each successful bidder will be required to furnish a satisfactory performance bond in the sum of the full amount of the contract. Bids shall include the furnishing and installation of all labor, materials, permits, equipment, tools, supplies, etc., for the 2023 Utilities Rehabilitation Program. \ Approximate quantities of work to be expected are as follows: 4,292 LF - 8" Dia. CIPP Liner \ 274 LF - 10" Dia. CIPP Liner \ 541 LF - 15" Dia. CIPP Liner \ 116 LF - 24" Dia. CIPP Liner \ 164 LF - 27" Dia. CIPP Liner \ 118 EA - Test & Seal Lateral Connection \ 5 LF - 8" Dia. CIPP Spot Liner \ 6 LF - 15" Dia. CIPP Spot Liner \ 2 EA - 8" Dia. Sanitary Sewer Spot Repair \ 4 EA - Lateral Cleaning from Mainline \ 11 EA - Sanitary Manhole Replacement \ 6 EA - 6" Gate Valve & Box \ 23 EA - 8" Gate Valve & Box \ 125 LF - 6" Dia. D.I. Water Main \ 105 LF - 8" Dia. D.I. Water Main \ 7 EA - Hydrant \ Together with appurtenant construction in all contracts. \ All bids must be prepared as described in the Instructions to Bidders and filed electronically via the online electronic bid service.
