The Project will provide improvements to North First Avenue from Hickory Trail to Stuart Court Intersection and generally include 9" PCC street reconstruction, 6" and 4" sidewalk replacement, 6" ADA curb ramp replacement, 6" driveway aprons, storm intake top and throat replacement, 6" sub-drain and cleanouts, and pavement marking. The City reserves the right to reject any or all proposals and also reserves the right to waive technicalities and irregularities. Each proposal shall be completed on a form furnished by the City and must be submitted in a sealed envelope. In addition, a separate sealed envelope shall be submitted containing a completed Bidder Status Form and a bid security executed by a corporation authorized to contract as a surety in the State of Iowa, in the sum of 10% of the bid. The successful bidder will be required to furnish a Performance, Payment, and Maintenance Bond in an amount equal to one hundred percent (100%) of the contract price and the form shown in Section 0510, said bond to be issued by a responsible surety approved by the City, and shall guarantee the prompt payment of all materials and labor, and also protect and save harmless the City from all claims and damages of any kind caused directly or indirectly by the operation of the contract, and shall also guarantee the maintenance of the improvement for a period of five (5) year(s) from and after its completion and formal acceptance by the City Council. Liquidated Damages: $500 per day
