The proposed material includes: Five (5) W30x116 - 50,000 PSI (or heavier) wide flange beams with shear connectors. The girder length shall be 60 feet. 18 - 50 ft pieces, 900 LF, of HP 10x42 Steel Piling, with pile point reinforcements each. Two C7x9.8 channels, one welded to each outside girder, shall be included for mounting of guardrail posts, as shown on the attached detail. Two C7x9.8 channels, one for each end of the deck as shown on the attached detail. C12x25 diaphragms and diaphragm connection plates as shown on the attached detail. Stay-in-place forms, shop cut to fit snugly between the shear connectors of adjacent girders. Sufficient form material shall be provided to accommodate a 6 inch overlap at the joints between sheets. The gage of the forms shall be sufficient to support a 7 inch concrete deck. Four C12x25 channels, each 24 feet 7 inches in length. Two 1/2" x 16" plates, each 24 feet 7 inches in length. Two C6x10.5 channels, each 24 feet 7 inches in length. Thrie beam guardrail, 4 end sections, systems for a 60-foot-long bridge shall be included as shown on the plans. Sheet piling 30' long, total horizontal coverage 54' each end bent approximately 3,240 S.F. total. Material specified shall be delivered on the job site no later than 60 days after the Notice of Award. We propose to perform the work described below, and as outlined in the Specifications and Drawings, for the price as indicated. We will comply with governmental regulations including prevailing wage rates. Our Bid Bond or Cashier's Check in the amount of 5% of our bid is attached hereto. On acceptance of our bid, we will furnish the County with a Performance Bond, certificates of Liability and Property Damage Insurance in the amount of $500,000.00 and proof of Workman's Compensation Insurance in accordance with statutory limits. Prior to final payment, we will furnish the County certificates that there are no liens for material or labor used in the work, and that the prescribed prevailing wage rates in this area have been paid to all workers. The bridge shall be completed within sixty (60) calendar days after execution of the contract by the County. Delays caused by inclement weather or unworkable conditions at the site will be considered in enforcement of this requirement. Should the Contractor not pursue the work with due diligence, the ANDREW COUNTY COMMISSION shall have the right to deduct $500.00 per calendar day, for each calendar day after the agreed scheduled completion, as liquidated damages. The right is reserved to reject any or all bids.
