Scope of Work A. The Contractor shall provide preventative maintenance service as specified herein. Work shall be bid as an annual lump sum amount, invoiced in arrears in four (4) equal payments. Work shall include being available to provide Emergency Services at an additional charge to the contract. B. Site Location and Represented by: Randy Hodge Supervisor II Northern Region / Indiana State Police 8500 E. 21st Street Indianapolis, Indiana 46219 Telephone: 317-501-8785 C. The Contractor shall furnish all supervision labor, material and tools required to maintain, in optimum operating condition, each device of the HVAC & Temperature Control Systems. Services shall include inspection, calibration, cleaning and lubrication pertaining to the efficient operation of the system as listed on the Preventative Maintenance Checklist attached. 1. Optimum operating condition is defined as that level of performance which provides comfort to facility occupants, meets applicable laws, codes, regulations meet or exceeds the equipment manufacturers' specifications and maximizes equipment life expectancy. 2. Repair and replacement of parts, components and devices contractor will provide quotation to perform work at additional cost. D. The minimum number of planned inspections shall be four (4) for one fiscal year spaced approximately ninety (90) days apart. One of the inspections will coincide with the start of the heating season and one inspection will coincide with the start of the cooling season. E. Each preventive work period shall be pre-scheduled and directed from a maintenance management program initiated and maintained by the Contractor. The program may be computer programmed or manually operated. A copy of this schedule shall be given to the Physical Plant Director (PPD) upon award of the Contract. F. All work shall be performed in the presence of an Institution Representative who shall sign documentation, provided by the Contractor, that the work is complete. The signature of the foreman of the crew performing the work shall appear on all site prepared documents. G. Within ten (10) days after each examination or Emergency Service, a report shall be furnished to the Physical Plant Director indicating the scope of work performed andconditions requiring further attention and the name of the Agency Representative present. Failure to provide the report may result in a delay or lack of payment by owner. H. The preventative maintenance and the responsibility of the Service Company shall include appurtenant devices and systems related to the automatic temperature controls located at any or all the sites.
