INTRODUCTION San Jose State University (SJSU) is seeking a C10 licensed electrical services firmtoperformMCC Replacement servicers ("Service Provider"). The successful Service Provider will provide specific services to SJSU related to the Duncan Hall MCC Replacement Project. PROJECT DESCRIPTION The MCC replacement project delivery method will be DBB for the Duncan Hall building. The estimated project schedule once the Engineering Services contract has been awarded is approximately one (1) year for Contractor performance. SCOPE OF WORK Service Provider shall provide the following services to SJSU. o The scope of work includes replacement of existing MCC's, intercepting branch circuits and extending to new and existing equipment. Provide and install new VFD's and breaker panels identified in the permitted construction plans (Exhibit C) in accordance with Division 1 - General Requirements (Exhibit A) and Section 26 05 00 - Electrical Basic Materials and Methods (Exhibit B). PROPOSAL REQUIREMENTS The proposal need not be voluminous, but shall provide sufficient information to allow the campus to evaluate the Service Provider's approach, experience, staff and availability. The proposer shall provide: 1. Cover / Transmittal Letter: This letter must be signed by a member of the firm with the authority to commit to a contract on behalf of the firm offering the proposal. 2. Firm Qualification Statement: Elaborate your firm's expertise and services on public works projects, and if applicable on other Universities or CSU's. Provide a brief description of the firm including the number of years the firm has been doing business in California. Provide an organization chart for managing and executing this contract. 3. Provide a schedule and management plan for the work to be performed. 5. Provide experience in projects of similar scope: a. List three (3) public works construction projects completed in the past three (3) years. Include the type of work, year completed, contract amount. b. Three (3) references with names, addresses, and telephone numbers. 6. Provide a project cost estimate for the overall Electrical services for the duration of the project, and include your firm's hourly rate schedule.
