All bids will be received at NCMC Administrative Conference Room on August 17th, 2023 at 1:00 pm for materials, and equipment required to fully complete projects listed below. Project will be awarded to the lowest qualified, responsible, and responsive contractor(s). When submitting the bid, each bid must be submitted in a sealed envelope containing all requested items in the bid check list. All bidders are will need to attend the pre'bid conferences as indicated below. Attendance at these conferences is mandatory. By submitting bids, bidders acknowledge responsibilities and familiarities with the project sites. 1. Blanchard Pharmacy Location: 5948 N. Market Street, Shreveport, LA 71107 Project Summary: The work includes the construction of a new pharmacy building, approximately 3,000 square feet, and associated site work. The building is wood framed with an EIFS and manufactured stone facade. Louisiana Uniform Bid Form, Completed Bid Bond or Bid Security AIA Document A 310 (Circular 570 as amended) 5% of total bid, Acknowledgement of Addenda, and name of firm as required by R.S.38:2212(B)(2). The following can be submitted at a later date by the apparent low bidder: 2. Completed Section 000320 Subcontractor, Supplier, and Manufacturer List, 3. Completed Section 000450 Non Collusion Affidavit, and Completed Section 000520 Safety Regulations Agreement Form. 4. (3) example projects with references for each project. 5. Copy of Louisiana Contractors License The bond shall be countersigned by a person who is under contract with the surety company or bond issuer as an agent of the company and who is licensed as an insurance agent in this State and who is residing in this State. Bidders Experience: Bidder shall have and provide references for three (3) projects of new construction and/or renovation of medical or pharmacy clinics. Bidders without previous experience and references shall be disqualified. Bids shall be accepted only from contractors who are licensed by the State of Louisiana for the classification of Building Construction. No bid may be withdrawn for a period of sixty (60) days after receipt of bids, except under the provisions of La.R.S. 38:2214. The successful bidder shall supply all required insurance with carrier(s) acceptable to North Caddo Medical Center. The Owner reserves the right to reject any and all bids for just cause. In accordance with La.R.S. 38:2212 (A)(1)(b), the provisions and requirements of this section, those stated in the Advertisement for Bids, and those required on the bid form shall not be considered as informalities and shall not be waived by any public entity
