Each bid must be made upon the prescribed forms, furnished with the Contract Drawings and Specifications, including the non-collusion affidavit and disclosure statement and must be accompanied by a Consent of Surety and a certified check, cashier's check or Bid Bond and, not to exceed $20,000.00. Such checks and Bonds shall be made payable to the Owner and will be held as a Guarantee that, in the event the Bid is accepted and a Contract awarded to the bidder, the Contract shall be duly executed and its performance properly secured. Further, the successful bidder must furnish the policies or Certificates of Insurance required by the Contract. In default thereof, said checks and the amount represented thereby will be forfeited to the aforesaid Owner as liquidated damages. Bids must be accompanied, in the case of corporations not chartered in New Jersey by proper certificate that such corporation is authorized to do business in the State of New Jersey. Bidders are required to comply with the requirements of P.L. of 1975, C.127 (NJAC 17:27) regarding Affirmative Action & Executive Order 11246 regarding Equal Employment Opportunity, as amended. All corporations and partnerships must comply with Chapter 33, of the P.L. of 1977, regarding disclosure of partners and stockholders. Each bid must be enclosed in a sealed envelope bearing the name and address of the bidder, addressed to the Owner and labeled for the GLEN AVE AND 5TH ST. ROAD IMPROVEMENTS. All bids shall be irrevocable, not subject to withdrawal and shall stand available for a period of sixty (60) days. A Pre-Bid meeting may be held based on the number of questions raised by the Bidders. The Owner Reserves the right to reject any bids, to waive informalities or irregularities in the bids received and to accept the bid from the lowest responsible bidder. The work to be performed under this contract includes improvements to Glen Avenue & Fifth Street in accordance with the plans and specifications. Principal Bid items of work in the project as estimated by the Engineer follows below. It is the contractor's responsibility to review the project and determine its own estimated quantities o 1 L.S. Traffic Control o 70 L.F. Sawcutting o 4,900 S.Y. H.M.A. Milling, 3" or Less, Full Width o 7 Units Reset Existing Casting o 4 Units Reset Utility Valve Box o 7 Unit Bicycle Safe Grate o 1 Unit Reconstructed Inlet Type B, Using New Casting o 30 Tons Hot Mix Asphalt 19M64 Base Course, Where Dir. o 650 Tons Hot Mix Asphalt 12.5M64 Surface Course, 2" Th. o 40 Gallons Prime Coat o 465 Gallons Tack Coat o 35 L.F. Traffic Marking Lines, 24" o 40 S.F. Traffic Marking Symbols o 200 L.F. Traffic Stripes, 4" o 11 S.F. Regulatory and Warning Sign o 32 M.H. Police Traffic Directors The work contemplated under this contract shall be completed within 60 calendar days from the date of execution ofthe contract.
