The purpose of the RFP is to find a licenced contractor to renovate the existing Lab / Immunization Clinic housed in the Siskiyou County Public Health building located at 810 S. Main St. Yreka, CA 96097. The space is approximately 416 square feet that requires replacement of outdated kitchen-style cabinets, countertop and sink. Deadline to Submit Questions 2/21/2023 Review of Proposals 3/2/2023 Public Works Contract Processed 4/3/2023(Estimated) Public Works Contract Start Date 5/1/2023(Estimated) Services the successful Proposer will be expected to provide, include but are not limited to: The renovation will include installation of medical-grade cabinets, countertops, and sink in the following estimated measurements: See Attachment A 1. 131 inch wide by 30 inch deep cabinet (1) and counter top with double sink (2); 2. 131 inch wide eye-level cabinets (3) with glass doors; 3. Matching style L-shaped cabinetry 152 inch by 72 inch (4); 4. Matching style L-shaped cabinetry 152 inch by 72 inch eye-level cabinets (5) with glass doors; 5. Desk-height medical-grade cabinets in bay window-shaped space 143 inches wide (6); 6. Desk-height island style rolling cabinet with medical grade counter top (7); 7. Carpet replaced with waterproof surface flooring for approximately 416 square feet (8). Completion of this project is required within 90 days of the Public Works Contract Start Date (contract acceptance).
