The County of San Bernardino (COUNTY) requires a Fountain maintenance service provider (CONTRACTOR) for the following purpose. Fountain Maintenance at: 1. County Government Center, 385 Arrowhead Road, San Bernardino. 2. Rancho Law & Justice, 8303 Haven Ave, Rancho Cucamonga. 3. Sheriff Coroners Office, 175 S. Lena Rd. San Bernardino. Maintain pumps and filters Remove all debris/trash/leaves Add chemicals to keep water clear Government center fountain maintained on Tuesday mornings before 9.00 AM 1. By submitting a bid, CONTRACTOR agrees to be bound by the terms and conditions of the "SAMPLE CONTRACT" (See Attachment A) if selected. CONTRACTOR agrees to comply with all terms set forth in attachment A, including but not limited to those provisions relating to Insurance and Indemnity. 2. CONTRACTOR must complete and submit Attachment B - Contractor's Cost Per Site. 3. A three year contract will be awarded. 4. All CONTRACTORS must meet the COUNTY insurance requirements prior to award of a Contract/Purchase Order. 5. Any Purchase Order issued pursuant hereto will incorporate this document and all terms contained herein. A. The CONTRACTOR shall provide all necessary labor, supervision, equipment and transportation to perform the highest degree of Fountain Maintenance. CONTRACTOR shall endeavor to maintain good public relations at all times. B. CONTRACTOR'S maintenance crew is to act immediately on any requests made by the onsite manager, the Ground Division Superintendent, or any one of the Landscape Services Team. C. All work conducted in a manner, which will cause the least possible interference and annoyance to the public and COUNTY employees, work performed by competent supervised employees and highly experienced in Fountain Maintenance. D. An emergency telephone, pager or other method of immediate communication or contact is required and submitted to Facilities Management for emergency contact. A. Purpose It is the intent of the COUNTY to maintain all COUNTY facilities at the highest level. In order to achieve this, periodic inspections are made. These inspections are intended to provide both the COUNTY and the CONTRACTOR information pertaining to the CONTRACTOR'S performance at each location. B. Quality Control Ratings The COUNTY may inspect CONTRACTOR'S performance at any time. In the event the COUNTY determines that the CONTRACTOR'S performance of its duties or other terms of the contract are deficient in any manner, COUNTY will notify CONTRACTOR of such deficiency orally, or in writing. If given orally, COUNTY shall provide written confirmation within five (5) days. CONTRACTOR shall remedy any deficiency within forty-eight (48) hours of such notification, or COUNTY, at its option, may terminate the contract immediately upon written notice or remedy the deficiency and offset the cost thereof from any amounts due CONTRACTOR under this contract or otherwise.
