ROOFING Project Name: 2022-080 Replacement of Roof Project Location: HC Solomon Mental Health Center 391 Varnum Avenue Lowell, MA 01854 Estimated Construction Cost: $730,000 Remove the existing roof substructure, install insulation with EPDM roof, install security fences with enclosures, and install underlying spray foam, and other items to complete the project. Minimum rates of wages to be paid on the project have been determined by the Commissioner of the Division of Occupational Safety under the provisions of Sections 26 and 27, Chapter 149 of the General Laws and will be included in the bid package. Proposals will ONLY be submitted online via COMMBUYS website by the General Bids Submission Deadline date on forms furnished by the Department of Mental Health (DMH) and clearly identified as a bid, endorsed with the name and address of the bidder, and the project name. Each general bid proposal must be secured by an uploaded photocopy of an accompanying deposit of 5% of the total bid amount, including all alternates, in the form of a bid bond, a certified, treasurer's, or cashier's check issued by a responsible bank or trust company made payable to the Commonwealth of Massachusetts. Awarded vendor will then mail the original 5% deposit bond or check to the Department of Mental Health Engineering and Facilities Management Department at 167 Lyman Street Room 158 Westborough, MA 01581.
