Engineer's Estimate: Greater than $1.5M but less than $3M ( Qualified Health Insurance Required) Davis-Bacon Wage Rate is Non-Applicable. Project Location: SR-163; Mexican Hat Arch Rehabilitation, 0C 274 The principal items of work are as follows (for all items of work see attachment): Cleaning and Repainting Structural Steel (23,000 sqft) Consultant 1 Traffic Control The project is to be completed: to be determined by competitive bid. All project bidding information, including Specifications and Plans, can be viewed, downloaded, and printed from UDOT's Project Development Construction Bid Opening Information online Project information can also be reviewed at the main office in Salt Lake City, its Region offices, and its District offices in Price, Richfield, and Cedar City As required, a contractor's license must be obtained from the Utah Department of Commerce. A prime contractor is subject to qualified health insurance requirements if the prime contract is in the amount of $2,000,000 or greater. A subcontractor is subject to qualified health insurance requirements if the subcontract is in the amount of $1,000,000 or greater.The Contractor shall demonstrate compliance with qualified health insurance requirements at the time of the execution of each initial prime contract. Each bidder must submit an electronic bid bond from an approved surety company using UDOT's Electronic Bid System (EBS); or in lieu thereof, cash, certified check, or cashier's check for not less than 5% of the total amount of the bid, made payable to the Utah Department of Transportation, showing evidence of good faith and a guarantee that if awarded the contract, the bidder will execute the contract and furnish the contract bonds as required.
