County : Summit This Project Requires Using the Electronic Certified Payroll Program. See Attachment to Notice to Contractors. This is a P&t (Price + Time) Project. Engineer's Estimate: Greater than $1.5M but less than $3M (Qualified Health Insurance Required) Project Location: SR-32; MP 21.00 - 29.05 The principal items of work are as follows (for all items of work see attachment): Fine Graded HMA (PG 64-34) - 1/2 inch Rotomilling - 3 Inch Mobilization The project is to be completed: to be determined by competitive bid . Project information can also be reviewed at the main office in Salt Lake City, its Region offices, and its District offices in Price, Richfield, and Cedar City. As required, a contractor's license must be obtained from the Utah Department of Commerce. Qualified Health Insurance is required on this project. Insurance must be submitted with executed contracts. See Standard Specification 00515 - 1.11(A). A prime contractor is subject to qualified health insurance requirements if the prime contract is in the amount of $2,000,000 or greater. A subcontractor is subject to qualified health insurance requirements if the subcontract is in the amount of $1,000,000 or greater. Each bidder must submit an electronic bid bond from an approved surety company using UDOT's Electronic Bid System (EBS); or in lieu thereof, cash, certified check, or cashier's check for not less than 5% of the total amount of the bid, made payable to the Utah Department of Transportation, showing evidence of good faith and a guarantee that if awarded the contract, the bidder will execute the contract and furnish the contract bonds as required.
