This project consists of removal and replacement of existing sidewalk and driveways including: installation of 13,083 square feet of 4" concrete sidewalk, 528 square feet 8" concrete sidewalk, 16 square yards 6" concrete driveway, 95 square yards 8" concrete driveway, 30 linear feet type 6 curb with truncated domes at crossings along Beech Street as indicated in the drawings and specifications. Questions regarding the work should be directed to the City Engineer at 740-393-9528. Each proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must submit evidence of its experiences on projects of similar size and complexity. The Owner intends and requires that this project be completed within ninety (90) days of issuance of notice to proceed. Each proposal must be accompanied by a bid guaranty in the amount of ten (10) percent of the bid. Said guaranty may be in the form of a bid bond, bid guaranty and contract bond, a certified check or cashier's check or a letter of credit. Bid guarantees must meet the requirements of Section 153.54 of the Ohio Revised Code. Bids shall be sealed and marked on the outside envelope as bid for Beech Street Sidewalk Improvements CDBG 2018 and mailed or delivered to the aforementioned address. All contractors and subcontractors involved with the project will, to the extent practicable use Ohio Products, materials, services, and labor in the implementation of this project. Additionally, contractor compliance with the equal opportunity requirements of Ohio Administrative Code Chapter 123, the Governor's Executive Order of 1972, and Governor's Executive Order 84-9 shall be required. Attention of bidders is called to all of the requirements contained in the bid packet, particularly the Federal Labor Standards Provisions and Davis-Bacon Wage Rates, various insurance requirements and equal opportunity provisions. The City reserves the right to reject any and all bids and to waive any informalities.
